"Another topical prescription I can try to help slow down oil production and get fewer of these lesions to grow is something called Winlevi. This is a topical acne medication that targets the oil glands specifically to reduce oil production so we don't feed the bacteria that cause acne. Topical Winlevi is very similar to oral spironolactone, which is commonly used in females for hormonal acne. Winlevi has to be used twice a day to be effective, and currently it's only a brand-name medication, meaning it's going to be expensive depending on your insurance plan."

However, there are significant limitations: it's very expensive at about $500 to $600 per tube, insurance often won't cover it, it requires twice-daily application which tends to hurt compliance, there isn't enough long-term data on potential HPA axis suppression with prolonged use, and I don't envision it as a standalone treatment but as an add-on because it doesn't address all components of acne. Tretinoin has additional benefits like improving collagen production and treating sun damage that this drug doesn't provide."
