What the gurus are saying

The gurus view Ultherapy as an effective skin tightening option, especially for those with facial fullness or jowling, and praise that it works equally well across all skin tones and skips the surface layer entirely, meaning no burning or scarring risk. However, the treatment can be genuinely painful since numbing creams cannot reach the deeper layers it targets, though pain management options like nitrous oxide have made recent procedures far more tolerable. The key caveat: results are gradual rather than dramatic, and inexperienced providers may accidentally break down facial fat, making the face look thinner; practitioners need skill to target the right muscular layer and avoid over-treating areas like the cheeks where volume matters.

"I really love Ultherapy because it doesn't break the skin barrier and it delivers ultrasound energy versus any sort of micro needling with radio frequency. I'm a little bit always more nervous because of my melasma, but I've done Ultherapy every 2 years for like the past few years or so. And even though the results are not dramatically wow, they have definitely helped over time to keep my face looking tight."

"I walked out of Ultherapy I think about 10 years ago but now I could get it without numbing so really, so huge, there's a huge change in like pain level. Ultherapy is best if you have a lot of fat. If you have a lot of facial fat and like jowling, people with facial fat benefit more from Ultherapy as well. You can customize the treatment to where if somebody doesn't have a lot of facial fat, you could just hit their lower face so that you kind of give them like a facelift result."

"Ultherapy is the most popular treatment that people come in for skin tightening. It uses an ultrasound machine that goes all the way down through the epidermis, dermis, subcutaneous tissue, muscle, and all the way up to the bone, tightening every single one of them so that you can avoid getting a facelift. You can use this skin tightening anywhere on the body. It's preventative medicine. If you want to keep your skin youthful and feel confident, this is what you should do. With proper pain management using Pronox, the laughing gas, the experience is completely different. I slept through an hour and a half of my treatment and did not feel a thing."

"There are some energy based devices that can actually target this region so they're ultrasound based devices, so high frequency ultrasound devices. One of the most famous is Ultherapy. It has to be used in the right way. If you go to someone who's not experienced with Ultherapy you can end up breaking down a lot of fat during the time period and you can actually look a lot worse. We want to target the SMAS layer, which is the muscular layer within the face, to cause contraction within that area."

"Ultherapy was sort of non-selective; it directed the ultrasound waves downwards so perpendicular, and in some cases destroyed a layer of fat which would have prematurely aged the face. We want to selectively target certain layers. The difference is that it's less precise, you don't know exactly the layer you're going in, whereas with newer technology you can measure that and direct the ultrasound energy to that specific layer so you can target that sweet spot where you get the collagen remodeling."

"If you're going to get skin tightening treatments utilizing some type of a heat-based treatment like Ultherapy, you may want to be careful with parts of the face like, let's say, the cheeks, in which you don't want to lose volume. I still have patients who undergo treatments with Ultherapy that tell me that their face looks thinner after these procedures are done. Be very careful with the upper part of the face."

5 Ways to Tighten Your Skin Without Surgery- Plastic Surgeon Reveals!
"Up until a few years ago I would consider Ultherapy to be the gold standard for non-invasive skin tightening. This treatment uses ultrasound energy to target the deeper skin to heat that skin up and cause that skin and the collagen to tighten. One great part of this treatment is that it is completely colorblind if you've got darker skin or if you have lighter skin it works the same for everybody. And another benefit of this treatment is that it bypasses the upper layer of the skin so it doesn't cause any burning or trauma to the surface of your skin. But that same benefit of bypassing the surface of the skin can also be a bit of a detriment because topical anesthetics numbing creams don't help to numb up this type of treatment. So there are a lot of people who've had Ultherapy and have complained that it really does hurt."
