What the gurus are saying

Gurus agree ketoconazole shampoo is evidence-based and effective for fungal scalp issues and dandruff, especially when left on skin for several minutes. The consistent caveat is that it can be drying, particularly for longer hair, and its impact is modest within a broader routine since it rinses off quickly. One reviewer noted it may not be sufficient as a solo solution for significant hair loss in older adults, though it can help in younger people or those with mild concerns.

"Ketoconazole is one of the most effective treatments for seborrheic dermatitis. It's an antifungal ingredient that works to get rid of excessive malassezia yeast and calm it down. These are solid evidence-based ingredients for reducing the burden of malassezia. Apply the shampoo to the affected area, lather it there, and let it sit on the skin for a few minutes, because in many cases patients simply get better results."

"The most effective in this category is going to be the ketoconazole shampoo. It comes as 1% over the counter. It works as an anti-androgen and not only does it help new hair grow by decreasing the inflammation, it is also keeping your scalp clear of yeast and buildup for optimal scalp health and hair growth."
Top Pick: anti-dandruff shampoo

"Ketoconazole has some data and that's probably the best shampoo you may be able to get in terms of data-based ingredients, but you're rinsing it off like how much is it doing? It's going to help but it's still probably the least impactful step in your routine."

"Ketoconazole shampoo is an anti-fungal medicine that can help your face, especially if you have dandruff on your face around your eyebrows, in the corners of your nose, or along your eyelid margins. When that subsides and you calm the seborrheic dermatitis down, you minimize the inflammation associated with it and your redness gets better in the long run. The shampoo is not necessarily the best as a face wash because it's not the ideal pH and it might be a little bit stripping, but if you use it several times periodically, maybe twice a week or four times a week, in the areas that are actually the worst, letting it sit for a few minutes before washing it off, you will see a huge difference in your skin in the long run. You're going to have to incorporate this throughout the course of your life and figure out the cadence of how you want to use it."

"These kind of shampoos are mild DHT blockers. If you're in your 30s this might be enough, right, but it's not enough for me. One of the downsides of these kind of shampoos is that it could lead to extremely dry hair. If you're a man and you've got short hair this might work much better for you. For a woman, for me, to get this in my scalp and leave it there for three to five minutes, it literally turns my hair into straw. It's an anti-fungal, it decreases dandruff and it reinforces the sebum level as well so it does help with the sebaceous gland which coats the hair. But you do need to do it two to four times a week and you need to leave it on for about three to five minutes."
