Sometimes we'll inject Botox in a V shape coming up the forehead to reduce the wrinkles of the central forehead, still leaving the eyebrows laterally moving up and down as they need to. If you're going to get Botox for the first time, one tip I would give you is to possibly consider avoiding the forehead. That takes a lot more skill to get a nice result. If you just want to try it and dabble in Botox, start with the glabella or with the crow's feet. You're much more likely to get a reasonable result that doesn't create too much of a change. In order for a muscle to contract, it needs the nerve to stimulate that muscle contraction. Botox prevents that from happening so that the muscles don't get that nerve signal and they basically don't contract. That's why it works well for dynamic wrinkles, wrinkles that are caused by muscles. Some examples are wrinkles of the glabella or the frown lines, the forehead and the crow's feet. "Botox, we say about 2 to four months so usually 3 months is usually where you sit out when you have a Botox and people come in every 3 months typically. Neuromodulators are quite safe especially if you have an injector who knows what they're doing. It's quite versatile we use it on those areas for dynamic wrinkles but also for areas where we don't want as much movement, say the neck. If you smile and activate the platysma muscles people worry about vertical cords and we can inject Botox into those vertical cords to help soften the platysmal bands and help when you make certain expressions or smile. A lot of my patients who see those vertical bands in photographs and are really bothered by them come in for Botox and we can inject those bands. But if you have someone who doesn't know what they're doing they can inject in the wrong area and cause either a facial droop or even, you know, there are reports of people having loss of voice, difficulty swallowing. That's very severe so always go to an experienced injector."

There's been a big push to do smaller treatments of Botox so we don't get that frozen look and that's what I love. I can still move my brows and scowl a little bit. I only get Botox now every 6 months that's what's comfortable for my budget and that's what's working for me. Some people get it every 3 months some people get it every 4 months. It's a treatment that you do have to keep up with obviously. If you're somebody who already has a line formed in the middle of your forehead because you frown so much but you start getting Botox let's say at 35 because that line already formed, when you hit 55 that line will not be as deep and it might not be apparent at all because you've prevented it from actually getting deeper over time."

"I don't really buy into the preventative Botox thing. The reason I am not such a huge proponent of it is that we don't really have studies to substantiate it. It's not an evidence-based recommendation. It's not like we have rigorous studies looking at the long-term ramifications of cosmetic neuromodulator injection over years and years and years. We don't have a lot of 70, 80-year-olds out there who have been getting neuromodulator for decades and decades to really see how it has ultimately impacted their overall face. A lot of the push for preventative Botox comes from small case reports, anecdotal patient observations, and clinician experience. Clinical experience is important, however it's also worth noting that cosmetic Botox is not medically indicated. It is a medical procedure, but it's not medically indicated. There's a financial incentive for the provider. Not saying that's the sole motivation for recommending preventative Botox, but it definitely is something worth pointing out."

"It's interesting though that patients report and clinical experience confirms that many patients who get regular Botox, not only do they have that improvement in dynamic wrinkles but they also have improvement in these other things like wrinkles that were already there at baseline. There are a couple of proposed mechanisms behind other impacts of Botox on the skin. One is something called a learned response, basically there's this idea that by getting regular neuromodulator injections you sort of learn to not recruit those muscle groups regularly or as aggressively because you spend time with them paralyzed, you kind of get out of the habit of doing things like that, that repetitive movement that creases the skin. But it doesn't explain how it is that wrinkles continue to improve after Botox wears off, there's evidence of persistent change to the..."

If you're wanting a more natural approach, you can use less Botox and that'll still give you free range of movement. I typically get injected in my forehead area and always get my glabella because I have one deep static wrinkle forming a vertical line there that's getting etched into my skin. I usually get three injections on each side around my crow's feet because I have fine wrinkles over there."
