What they liked

  • Lotion base less drying than generic tretinoin
  • Spreads easily across the face
  • Lasts a long time per bottle

What they flagged

  • Significantly more expensive at some retailers
  • Still causes peeling despite hydrating base
  • Irritating for some people despite formulation

What the gurus are saying

The hydrating base noticeably cuts down on peeling and irritation compared to standard tretinoin, though some people still find it irritating regardless. Gurus praise how the gel-like texture spreads smoothly across the face, especially when applied over a hydrating toner underneath, and it does require only a pea-size amount to cover the whole face. The main trade-off is cost: it's considerably more expensive than generic tretinoin lotion, so whether the gentler formula is worth the price depends on how your skin tolerates standard versions.

"I cycle back to Altreno, which is a prescription tretinoin."

"Your dermatologist might try to prescribe other topical retinoids in vehicles that help reduce irritation, such as Altreno lotion, which contains hyaluronic acid and collagen. It is often better tolerated, it's a lot more moisturizing, and it's formulated to really address the dryness and sensitivity issues, but there's no guarantee with that."

Tretinoin Causes Death & Is Banned?
"We now have a newer form of tretinoin called Altreno lotion, it is in a much more like moisturizing hydrating base, it has hyaluronic acid, it has collagen. People seem to do better with that, but I also have had patients say this is very irritating, I don't like it. So I don't know if a lot of that is just the drug companies hyping up the base. In Altreno lotion it's also significantly more expensive."

"Altreno lotion, a topical retinoid is formulated with hyaluronic acid and collagen to be more hydrating, more moisturizing, and it is allegedly a lot less drying and irritating."

"Altreno lotion is a newer form of tretinoin, tretinoin in a lotion, and it's a lot less drying and irritating. It's formulated with hyaluronic acid, so it's safe to use."

"I like Altreno. Do you know about Altreno? You know all my derm friends tell me to do it and then I never get on it cuz I'm scared. It's a brand name tretinoin. It's a lotion instead of a cream and it's formulated with collagen, glycerin, hyaluronic acid, I think some really great ingredients. So it's just a little bit more gentle."

"Altreno lotion, this is the 0.05%. It's a little bit newer hydrating formula. Really well tolerated. I do have this prescription. I like it. It spreads really nice."

How a Dermatologist Transitions Her Skincare for Spring & Summer | Dr. Aleksandra Brown
"If you've been wondering, there's also Altreno. This is a brand name. I'll tell you, my insurance did not cover this. I paid probably over 100 bucks for this, too. But it lasts a very long time. So, if you're somebody who wants that gel texture, but you don't want to be too irritated with the generic tretinoin, which is tretinoin lotion. It feels like gel. I know it says lotion, but it feels just like gel. Is very nice option, a very good alternative. I apply this directly on top of my serum at bedtime, and then I do moisturizer on top."
Dermatologist's Winter Skincare Routine: AM & PM Steps for Healthy Skin | Dr. Aleksandra Brown
"I also have the Altreno lotion, which is the brand name of the tretinoin gel. It does help with some of the irritation, but you can still experience peeling whenever you use your tretinoin regardless of whether you're using like a retinol over the counter or the actual tretinoin. You only want to use like a pea-size amount and cover your entire face. So I like having the toner underneath because it really spreads the cream nice. Sometimes the cream is a little bit harder to spread, and having that hydration underneath helps the spread much, much easier."
Retinol, Retinal, Tretinoin: What’s the Difference?
"Altreno lotion, this is the 0.05%. It's a little bit newer hydrating formula. Really well tolerated. I do have this prescription. I like it. It spreads really nice."
Yay
"I love the Altreno lotion. This is prescription. So, I can't link to it below, but um I just had my regular doctor write out a prescription for it and I had it filled at the pharmacy and just paid um I think I went to Costco for this one. Walgreens was really expensive, but Costco is actually ridiculously cheap. And this one's nice. So, if you have dry skin, this is a nice prescription retinoid that doesn't dry your skin out like a lot of them otherwise the prescription ones can. uh they some of them contain a lot of denatured alcohol which can really dry your skin out in combination with the retinoids making it even drier but this one's really nice at preventing that."
Yay
"There are some fancier versions like Altreno which has that nicer lotion base it's less irritating"
Yay
"It's called Altreno, A-L-T-R-E-N-O. It is a prescription strength tretinoin that is formulated with other types of ingredients that helps ease the irritation. So when you go to your dermatologist, if you can't make it to Mexico or you don't live in a city where tretinoin is not prescription or is only available in prescription, then you can ask your dermatologist if they can prescribe Altreno, which is a prescription tretinoin with a lot less irritation because of the other ingredients and the formula that they make it."
Yay
"This is the Altreno and it is tretinoin .05 and a really hydrating nice base. It's got glycerin in here, they also have hyaluronic acid as well as a soluble collagen. It just really kicks everything up a notch. I'm using just one pea-sized amount and that pea-sized amount is going to be distributed evenly all over my face. I [...] have now graduated to a prescription strength, but trust me it took me years to get to this point. [...] You absolutely have to wear sun protection in the morning anytime you're using retinol at night time. We know that retinoids or any type of topical vitamin A can be irritating to the skin, so pairing it with the right moisturizer is so important."
Nay
"The most recent retinoids I've been using are prescription. I was using Altreno, which has the most elegant hydrating jelly texture. I had to stop using that because I felt the formula, unfortunately as beautiful as it was, it's just a little too strong for my skin even after six months."

What the evidence says.

Independent of what the gurus think, this is what the published research supports about the main active ingredients here.

Humectants and emollients

Ingredients like these draw water into the surface layers and slow how fast it leaves. The evidence is for comfort, softness and hydration rather than for changing the structure of the skin.
