What the gurus are saying

Experts praise Altreno's gel-like texture and hydrating base for easing the irritation that often comes with tretinoin, making it a gentler alternative to the generic version. The trade-off is cost: it carries a significantly higher price tag. That said, response splits notably. While some patients report better tolerance, others find it equally irritating, suggesting the fancier base may not guarantee smoother results for everyone. Those starting tretinoin may want to apply just a pea-size amount over a hydrating toner to help it spread evenly and minimize peeling.

"Altreno is a brand name. It lasts a very long time. If you want that gel texture but you don't want to be too irritated with the generic, which is tretinoin lotion, it feels like gel. I know it says lotion, but it feels just like gel. It's a very nice option, a very good alternative. I apply this directly on top of my serum at bedtime, and then I do moisturizer on top."

"I also have the Altreno lotion, which is the brand name of the tretinoin. It does help with some of the irritation, but you can still experience peeling whenever you use your tretinoin regardless of whether you're using like a retinol over the counter or the actual tretinoin. You only want to use like a pea-size amount and cover your entire face. I like having the toner underneath because it really spreads the cream nice. Sometimes the cream is a little bit harder to spread, and having that hydration underneath helps the spread much much easier."

"There are some fancier versions like Altreno which has that nicer lotion base. It's less irritating."

"We now have a newer form of tretinoin called Altreno Lotion. It is in a much more moisturizing, hydrating base. It has hyaluronic acid, it has collagen. People seem to do better with that, but I also have had patients say this is very irritating, I don't like it. So I don't know if a lot of that is just the drug companies hyping up the base. Altreno Lotion is also significantly more expensive."
